When someone with a fart fetish (one who is erotically & sexually fixated with farts/farting) fantasizes and/or masturbates to their object of fixation farting, they do not focus on the stench of the fart but of the noise made by the person passing gas, both from the anus and the erotic moaning they may add to simulate extreme pleasure during the act. Hence the name - "fantasy fart": there's no way you can't smell somebody passing gas - it is, more or less, a "fantasy".
